CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that BMPEA strongly inhibits CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo pill: the codeine part is a prodrug. Your body needs an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Lab studies suggest BMPEA strongly blocks that same enzyme. So in theory, less of your codeine would get converted, meaning you might feel weaker pain relief than expected.
I'll be honest, this is theoretical and based only on test-tube data, so I wouldn't lose sleep over it. The bigger concern is BMPEA itself. It's a hidden amphetamine-like stimulant that can spike your heart rate and blood pressure, and stacking that with caffeine and butalbital isn't a great mix. I'd skip the BMPEA and chat with your pharmacist or doctor first.

Here's the gist for your Fioricet with Codeine combo. In lab dishes, BMPEA appears to slow down an enzyme called CYP3A4 by a little over a third. That same enzyme helps your body process parts of this medicine, including the butalbital. If that breakdown is blocked, levels could drift a bit higher, which might mean more drowsiness, dizziness, slowed breathing, or feeling foggy. There's also an odd push-pull here: BMPEA is a stimulant that revs up heart rate and blood pressure, while this pill tends to sedate you, so the two work against each other in uncomfortable ways.
One caution worth flagging: this is only a theoretical concern from test-tube data, not proven in people. Even so, mixing an unregulated amphetamine-like supplement with a sedative opioid product isn't something I'd recommend. Please check with your pharmacist or doctor first.

Here's the thing to focus on with this combo product: it contains caffeine, which is itself a stimulant. BMPEA is a synthetic, amphetamine-like stimulant that can push up your heart rate and blood pressure. Stack a stimulant supplement on top of the caffeine in Fioricet with Codeine and you could feel more jittery, anxious, and notice a racing or pounding heartbeat. In rare cases, that added cardiovascular strain is the real worry.
To be fair, this concern is theoretical, based on how these ingredients behave rather than on solid human studies. But BMPEA isn't a well-vetted ingredient and regulators have flagged it, so I'd lean toward avoiding it. Please check with your pharmacist or doctor before mixing the two.

Beta-methylphenethylamine (BMPEA) is a synthetic stimulant chemically similar to amphetamine that has been found in some weight-loss and sports supplements, often without being clearly listed on the label.
