Why this applies to Riociguat
Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.
In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Adempas (riociguat). Your body clears this medicine partly through a liver enzyme called CYP3A4. In lab testing, BMPEA appears to slow that enzyme down a bit (around a third). If that happens in real life, riociguat could build up higher than expected, which may exaggerate its blood-pressure-lowering effect. That can mean dizziness, lightheadedness, fainting, headache, or an upset stomach, especially when you stand up.
I'll be honest, this is a theoretical concern based only on test-tube data, not on real patient studies. But there's a bigger issue: BMPEA is an amphetamine-like stimulant that can raise blood pressure and heart rate, which works against what riociguat is trying to do for your lungs. I'd steer clear of it and check with your doctor or pharmacist first.

Beta-methylphenethylamine (BMPEA) is a synthetic stimulant chemically similar to amphetamine that has been found in some weight-loss and sports supplements, often without being clearly listed on the label. It is not an approved or well-studied dietary ingredient, and regulators have warned that it may carry real cardiovascular risks. About Riociguat
What Is Riociguat Used For?
Brand name: Adempas
Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).
The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
