There is some concern that electroencephalogram (EEG) biofeedback might lower the seizure threshold in some patients. Theoretically, combining EEG biofeedback with seizure threshold-lowering drugs might increase the risk of seizure.
Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the thing to keep in mind with clozapine: it already has a known tendency to lower the seizure threshold, meaning it can make seizures more likely, especially at higher doses. EEG biofeedback (a specific type that trains brainwave activity) has a theoretical concern of doing the same thing in some people. Stacking two things that nudge in that same direction could, in theory, add up.

I'll be honest, this is based on limited evidence and is largely theoretical, so it's not a reason to panic or avoid biofeedback. Most relaxation-style biofeedback (heart rate, muscle tension) isn't the worry here. If you're considering EEG neurofeedback while on clozapine, just loop in your prescriber or pharmacist first so they can weigh it for your situation.

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
