Theoretically, bitter almond oil may have additive effects with other CNS depressants.
Bitter almond and its oil contain amygdalin, which is hydrolyzed to toxic hydrocyanic acid. This can cause CNS depression and respiratory failure.

Here's the simple version: meperidine already slows down your brain and breathing as part of how it relieves pain. Bitter almond contains amygdalin, which the body can turn into cyanide. In concentrated amounts, that toxic compound can also depress the nervous system. Put the two together and the worry is they could add up, leaving you with too much sedation, slowed breathing, dizziness, or confusion.

To be clear, this concern is theoretical and based on how these substances work, not on real-world reports in people. Still, bitter almond is genuinely toxic and isn't safe to take internally at all. If you want almond's benefits, stick with ordinary sweet almonds, and check with your pharmacist or doctor before combining anything with meperidine.
