CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

This combination product already contains phenylephrine, a decongestant that nudges up your heart rate and blood pressure. Bitter orange brings its own stimulant, synephrine, to the table. So my bigger concern here isn't really the liver enzyme issue, it's stacking two stimulants together, which could leave you feeling jittery, give you a racing or pounding heartbeat, or push your blood pressure higher.
On the enzyme side, bitter orange may mildly block CYP3A4 in your gut, which could slow the breakdown of some ingredients and let levels run a little higher. The evidence for that is fairly weak and mostly from fresh juice, not capsules. Either way, if you have high blood pressure or heart issues, please check with your pharmacist or doctor before combining these.

Here's the thing to watch with this combination: one of the ingredients in Rhinogesic is phenylephrine, a decongestant that works by tightening blood vessels. That can nudge your blood pressure up and make your heart work a little harder. Bitter orange contains synephrine, a mild stimulant that acts in a similar way. Stack the two together and those effects can add up.
For most people the worry is feeling jittery, a racing or pounding heartbeat, or a bump in blood pressure. This concern is largely theoretical and based on limited evidence, so it's not a reason to panic. Still, if you have high blood pressure or any heart issues, it's worth running this combo past your pharmacist or doctor first.
