Here's the thing to watch with this combo. Your Dimetapp DM contains phenylephrine, a decongestant that already nudges up your blood pressure and heart rate by tightening blood vessels. Bitter orange has synephrine, which is its own mild stimulant working in a similar direction. Stack the two together and those effects can add up, so you might notice a racing or pounding heartbeat, jitteriness, headache, or a bump in blood pressure.
This is a theoretical concern based on limited evidence, not something guaranteed to happen, and a normal cold-medicine dose is usually fine for most healthy people. But if you have high blood pressure, heart trouble, or take other stimulants, please run it by your pharmacist or doctor first before combining them.

DEXTROMETHORPHAN (Robitussin DM, others)
2 of 3One small clinical study shows that bitter orange juice increases dextromethorphan levels, likely through cytochrome P450 3A4 (CYP3A4) inhibition. Theoretically, bitter orange might increase the risk for dextromethorphan-related adverse effects.

Here's the practical takeaway for your Dimetapp DM. One small study found that bitter orange juice can slow down an enzyme called CYP3A4, which is one of the ways your body clears the dextromethorphan (the cough suppressant) in this combo product. When that pathway is blocked, dextromethorphan can build up a bit more than usual, which may mean more drowsiness, dizziness, or feeling foggy or jittery. Keep in mind the phenylephrine (a decongestant) and bitter orange's synephrine are both mild stimulants, so taking them together could also nudge your heart rate or blood pressure up.
This is based on limited evidence and a small study, so it's a possible concern rather than a sure thing. Still, if you take a bitter orange supplement, it's worth a quick chat with your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the quick version: Dimetapp DM contains dextromethorphan (the cough suppressant) and brompheniramine (an antihistamine), both of which your liver clears partly through an enzyme called CYP2D6. A compound in bitter orange may weakly slow that enzyme down in lab tests, which in theory could let a little more of these ingredients build up. If that happened, you might feel extra drowsy or dizzy, or notice more dry mouth.
But I want to keep this in perspective. This effect has only been seen in a test tube, never in actual people, so it's considered minor and mostly theoretical. There's also a stimulant side to bitter orange that can nudge up heart rate and blood pressure, so if you have heart concerns, check with your pharmacist before combining them.
