CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Citalopram
Citalopram falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: bitter orange may block an enzyme in your gut called CYP3A4, and citalopram (Celexa) is partly broken down by that same enzyme. If that breakdown slows, a bit more citalopram could end up in your system. In practice that might mean more of the usual citalopram side effects, like drowsiness, nausea, or feeling jittery. The bigger concern is citalopram's known effect on heart rhythm (QT prolongation), and since bitter orange's synephrine can nudge up heart rate and blood pressure on its own, the combination isn't ideal.
That said, the evidence here is limited and the effect size is uncertain. I wouldn't panic, but I also wouldn't stack a stimulant supplement on top of this antidepressant without checking. Please run it by your pharmacist or doctor first.

Citalopram — available as tablets (including brand Celexa), oral solution, and capsules — is used to treat major depressive disorder (MDD) in adults. This includes depression that causes persistent low mood, loss of interest in everyday activities, changes in sleep or appetite, fatigue, and difficulty concentrating.
Citalopram capsules are specifically indicated for adults with MDD but are not intended for starting or stopping treatment — another citalopram product must be used for dose titration. All forms are indicated for adults only; citalopram tablets are not approved for use in children.
