Here's the thing to watch with CoActifed: it already contains pseudoephedrine, which is a stimulant decongestant that can bump up your heart rate and blood pressure on its own. Bitter orange supplements contain synephrine, another stimulant that works in a similar way. Stacking the two together is like pressing the same gas pedal twice, so you could see a faster or pounding heartbeat, jitteriness, headache, trouble sleeping, or higher blood pressure.

This concern is mostly theoretical and rated moderate, so it isn't a guaranteed problem, but it's a real one worth respecting if you have heart issues or high blood pressure. Before pairing bitter orange with CoActifed, please check with your pharmacist or doctor first.

Theoretically, bitter orange might increase levels of drug metabolized by CYP2D6.
In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the wrinkle with this combo (CoActifed contains codeine, pseudoephedrine, and triprolidine). One compound in bitter orange may weakly slow down an enzyme called CYP2D6. Codeine is actually a prodrug, meaning your body uses CYP2D6 to turn it into morphine, which is what relieves pain and cough. So if that enzyme is dialed down, you could theoretically get less relief from the codeine, not more.

Separately, both bitter orange and the pseudoephedrine here are stimulants, so stacking them could nudge up your heart rate and blood pressure. That said, this enzyme effect has only been seen in lab dishes, never in real people, so it's very theoretical. Still, it's worth a quick chat with your pharmacist before combining them.
