CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Dextromethorphan, Guaifenesin, Pseudophedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the thing to keep an eye on with this combo. Bitter orange can mildly block an enzyme in your gut called CYP3A4, which helps break down the dextromethorphan part of this cough and cold product. If that breakdown slows a bit, dextromethorphan levels could climb, leaving you feeling more drowsy, dizzy, or foggy than usual.
The bigger concern, honestly, is overlap. Bitter orange is a stimulant, and this product already contains pseudoephedrine, which is also a stimulant. Stacking the two could nudge your heart rate and blood pressure up, causing jitteriness or a racing heart. The CYP3A4 evidence here is fairly weak, but if you have heart issues or high blood pressure, please check with your pharmacist before combining them.

Here's the thing to keep in mind with this combo cough and cold product. Your medicine has three parts: dextromethorphan (the cough suppressant), guaifenesin (to loosen mucus), and pseudoephedrine (a decongestant). The interaction on file is with the dextromethorphan piece. Bitter orange appears to slow down a liver enzyme called CYP3A4 that helps clear dextromethorphan, so the drug can build up a bit higher than usual. A small human study backs this up, so it's worth noting, but it isn't a huge red flag.
If levels climb, you might feel more drowsy, dizzy, or notice nausea or a foggy head. I'd also gently point out that bitter orange is a stimulant, and your product already has pseudoephedrine, so together they could nudge your heart rate and blood pressure up. Please check with your pharmacist or doctor before pairing them, especially if you have heart or blood pressure concerns.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist for your cough and cold combo. One of its ingredients, dextromethorphan, is cleared by a liver enzyme called CYP2D6. Lab studies suggest a compound in bitter orange might weakly slow that enzyme down. In theory, that could let dextromethorphan build up a bit, which might mean more drowsiness, dizziness, or a foggy feeling.
That said, this is only seen in test-tube research, not in real people, so I wouldn't lose sleep over it. The bigger concern is the overlap of stimulants: bitter orange (synephrine) and the pseudoephedrine in your product can both nudge up your heart rate and blood pressure. Stacking them could leave you jittery or with a racing heart, so please check with your pharmacist before combining them.
