Alogliptin falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the gist for your Nesina (alogliptin): psyllium is a gel-forming fiber, and when it sits in your gut it can grab onto pills and shift how much of the medicine actually gets absorbed. With alogliptin specifically, this hasn't been formally studied, so the concern is mostly theoretical and rated as minor. If absorption dipped a bit, the worst-case is slightly less help with your blood sugar control, not a dangerous reaction.

The easy fix is timing. Take your alogliptin first, then wait about 30 to 60 minutes before your psyllium dose. That spacing keeps the fiber from interfering. Keep checking your blood sugars as usual, and if you notice your numbers creeping up, mention it to your pharmacist or doctor before making any changes.

Blond psyllium is a soluble fiber from the seed husks of Plantago ovata, and it is one of the better-studied fiber supplements. It can help with both constipation and diarrhea and may modestly lower cholesterol and blood sugar. Alogliptin tablets — sold under the brand names Alogliptin and Nesina — are used along with diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
