Bazedoxifene Acetate, Conjugated Estrogens falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version: psyllium is a gel-forming fiber, and when it sits in your gut at the same time as a pill, it can grab onto that pill and change how much your body soaks up. With Duavee (which combines conjugated estrogens with bazedoxifene), the fiber could either slightly raise or slightly lower how much estrogen gets absorbed. Interestingly, animal data actually showed psyllium increasing absorption of a related estrogen, so the direction isn't fully predictable.

The good news is this is rated a minor, theoretical concern, not a dangerous one. An easy fix is simple timing: take your Duavee first, then wait 30 to 60 minutes before your psyllium. If you have questions, your pharmacist can confirm the best schedule for you.

Theoretically, taking blond psyllium at the same time as ethinyl estradiol might alter levels of estradiol.
Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. However, this is unlikely to be clinically significant.

Here's the short version: there's very little to worry about here. Duavee contains conjugated estrogens, which act in your body much like the estrogen component this interaction note is based on. The concern is that the soluble fiber gel from psyllium can grab onto things in your gut and gently slow how fast the estrogen is absorbed. Interestingly, the lab data suggests it might even nudge the total amount absorbed up a touch, not down.

The key word, though, is theoretical. This is based only on test-tube and animal work, and it's rated as minor and unlikely to matter in real life. You shouldn't notice any change in your hot flashes or any new side effects. To play it safe, you can simply separate them by a couple of hours, and check with your pharmacist if you have questions.

What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
