Pretomanid falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version: psyllium forms a thick gel in your gut, and that gel can grab onto pills and slow or change how much medicine your body actually absorbs. With pretomanid, this is mostly a theoretical concern. We don't have solid studies showing a real problem, so I wouldn't lose sleep over it, but pretomanid is a key part of your TB regimen and you really want it fully absorbed so it can do its job.

The easy fix is timing. Take your pretomanid first, then wait about 30 to 60 minutes before your psyllium dose. That spacing keeps the fiber from interfering. Since this drug is treating a serious infection, it's worth running your routine past your pharmacist or doctor so nothing gets in the way of treatment.

Blond psyllium is a soluble fiber from the seed husks of Plantago ovata, and it is one of the better-studied fiber supplements. It can help with both constipation and diarrhea and may modestly lower cholesterol and blood sugar. It is generally well tolerated when taken with plenty of water, but always start low and check with your pharmacist or doctor first.

About Pretomanid

What Is Pretomanid Used For?

Brand name: Pretomanid

Pretomanid Tablet is used only as part of a three-drug regimen — together with bedaquiline and linezolid — to treat adults with pulmonary (lung) tuberculosis that is resistant to multiple antibiotics, including isoniazid, rifamycins, a fluoroquinolone, and a second-line injectable TB drug. It is also used for adults whose TB is resistant to isoniazid and rifampin and who cannot tolerate or are not responding to standard TB therapy.

Pretomanid Tablets are not for drug-sensitive TB, latent (inactive) TB infection, TB outside the lungs, or TB that responds to standard treatment. They should only be used in combination with bedaquiline and linezolid — safety and effectiveness with other drug combinations have not been established.
