In vitro research shows that Bulbine natalensis stem extract slightly induces CYP1A2 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP1A2 substrates; however, this has not been studied in humans.

This combination pill pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). The liver enzyme CYP1A2 helps clear amitriptyline, and lab research hints that Bulbine natalensis might gently speed that enzyme up. In theory, that could push your body to break amitriptyline down a little faster, which might mean a weaker effect on your mood, sleep, or nerve pain.

I want to be straight with you: this is only a test-tube finding, and it has never been studied in real people, so the actual impact may be small or none. What concerns me more is that Bulbine natalensis has raised liver and kidney safety questions in animals. Before pairing these, please check with your pharmacist or doctor.

Here's the gist for your Triavil (which combines amitriptyline, an antidepressant, with perphenazine, an antipsychotic). In lab studies only, Bulbine natalensis appeared to speed up an enzyme called CYP2C9. If that played out in people, it could clear part of this medication a little faster, which in theory might make it work less well. For an antidepressant, that could mean your mood symptoms aren't as well controlled.

I want to be honest though: this is only a theoretical concern from test-tube and animal work, never confirmed in humans, so I wouldn't lose sleep over it. The bigger issue is that Bulbine itself has raised liver and kidney safety questions. Please check with your pharmacist or doctor before adding it, and never adjust your medication on your own.

Here's the gist with this combo. Your medication Triavil pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic), and both rely on liver enzymes to be cleared from your body. In lab dishes, Bulbine natalensis sped up an enzyme called CYP3A4. If that happened in people, it could in theory clear these drugs out a bit faster, meaning they might work less well. For amitriptyline that could look like depression creeping back, and for perphenazine, less control of your symptoms.
