Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your combo pill that pairs diltiazem (a blood pressure and heart rate medicine) with enalapril. The diltiazem part is cleared by a liver enzyme called CYP3A4. Lab and animal studies suggest Bulbine natalensis may speed that enzyme up, which could break diltiazem down faster and leave less of it working in your body. In plain terms, your diltiazem might become a little weaker, so your blood pressure or heart rate could creep back up.
I'll be honest: this is only a theoretical concern from test-tube data, not something proven in people, so I wouldn't panic. Still, given the supplement's possible liver and kidney risks, please check with your pharmacist or doctor before adding it.

P-GLYCOPROTEIN SUBSTRATES

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Teczem combines diltiazem (a calcium channel blocker) with enalapril (a blood pressure medicine). The diltiazem part is moved around the body partly by a transporter called P-glycoprotein. The concern here is theoretical and comes only from lab studies, where Bulbine natalensis affected the gene tied to that transporter. The lab work hinted it might induce (speed up) the transporter, which could push a bit more diltiazem out and possibly soften its effect, but honestly the evidence is too thin to predict the real direction.
Practically, watch for changes in how your blood pressure readings look. Given the weak data plus this plant's possible liver and kidney concerns, please run it by your pharmacist or doctor before adding it.
