Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP2C9 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP2C9 substrates; however, this has not been studied in humans.

Here's the gist for piroxicam (Feldene). Your body clears this anti-inflammatory mainly through a liver enzyme called CYP2C9. In lab dish studies, Bulbine natalensis seemed to speed up that enzyme. If the same thing happened in people, your body might break piroxicam down a bit faster, which could leave you with weaker pain and stiffness relief from your arthritis.

I want to be honest: this is only a theory from test-tube work, not something proven in humans, so the real-world effect may be small or none at all. Still, given the liver and kidney safety concerns around Bulbine itself, I'd run it past your pharmacist or doctor before combining the two rather than guessing.

Theoretically, combining Bulbine natalensis leaf with drugs that have anticoagulant or antiplatelet activity might increase the risk of bruising and bleeding.
In vitro research shows that Bulbine natalensis ground leaf extract inhibits platelet aggregation.

Here's the thing with piroxicam: like other NSAIDs, it already makes your platelets a little less sticky, so it can raise your odds of bruising, stomach irritation, and bleeding on its own. Bulbine natalensis appears to do something similar. In lab (test tube) studies, its leaf extract slowed platelets from clumping together, the very step that helps you stop bleeding.

So stacking the two could, in theory, add up and make easy bruising, nosebleeds, or stomach bleeding a bit more likely. I want to be honest though: this concern is theoretical, based on how each works rather than real-world reports. Still, NSAIDs and bleeding are a known pair, so please run this combo by your pharmacist or doctor first, and watch for unusual bruising, dark stools, or bleeding that won't quit.
