Acetaminophen, Caffeine, Pyrilamine falls under DIURETIC DRUGS — the group Calotropis interacts with.

Here's the thing to know about this one: the interaction in our database is really about potassium-depleting diuretics, which are water pills that flush potassium out of your body. Low potassium can make the heart-acting compounds (cardiac glycosides) in calotropis more dangerous. Your product, the Midol-type combo of acetaminophen, caffeine, and pyrilamine, is mainly a pain reliever with an antihistamine and a little caffeine. It is not a true potassium-wasting diuretic, so this specific warning doesn't fit it well.
That said, calotropis itself can be toxic to the heart and is not something I'd recommend taking internally. If you're using it at all, please loop in your pharmacist or doctor first. This concern is theoretical, so don't panic, but do check before mixing them.
