Theoretically, concomitant use of calotropis and potassium depleting diuretics can increase the risk of cardiac glycoside toxicity due to potassium depletion. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.

Here's the thing to know about your medication: it's a combo pill. The hydrochlorothiazide side can lower your potassium, while the amiloride side is actually potassium-sparing, meaning it helps hold potassium in. Calotropis is interesting because it contains natural cardiac glycosides, heart-acting compounds similar to digoxin. When potassium runs low, the heart becomes more sensitive to these glycosides, which raises the risk of toxic effects like an irregular or dangerously slow heartbeat, nausea, or dizziness.

I'll be straight with you: this concern is theoretical, based on how the chemicals behave, not on solid human studies. Honestly, the bigger worry is that calotropis itself can be poisonous and shouldn't be taken internally. Please don't change anything on your own. What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
