Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist with your headache medicine (Esgic, Fiogesic and similar combos). The butalbital piece, a barbiturate, is partly cleared by a liver enzyme called CYP3A4. In lab and test-tube studies, cannabis appears to gently slow that enzyme down. In theory, that could let butalbital linger a bit longer, which might add to its sedating effects, things like extra drowsiness, dizziness, slowed breathing, or feeling foggy. Both cannabis and butalbital can make you sleepy on their own, so combining them tends to stack that effect.
Keep in mind this is weak, theoretical evidence, not something proven in people. Still, before mixing the two, it's worth a quick chat with your pharmacist or doctor, especially about the combined drowsiness.

In vitro research shows that cannabis can induce the activity of CYP2E1, which might increase the metabolism of CYP2E1 substrates.

Here's the gist for this headache combo. The piece that matters is the acetaminophen (the Tylenol part). Your liver uses an enzyme called CYP2E1 to handle some acetaminophen, and lab studies suggest cannabis might speed that enzyme up. In theory, that could shift more acetaminophen down a pathway that produces a toxic byproduct, which is the same byproduct your liver normally has to neutralize.
I want to be honest though: this is based only on test-tube and animal research, so we don't really know how much it matters in real people. The practical takeaway is to be careful not to exceed your daily acetaminophen limit and to go easy on alcohol. If you use cannabis regularly and take this regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

BARBITURATES

Some research shows that synthetic delta-9-tetrahydrocannabinol (THC) increases the elimination half-life of pentobarbital by 4 hours when dosed concomitantly.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of this combo to watch here is the butalbital, which is a barbiturate. It's the sedating, muscle-relaxing part that calms a tension headache. There's a theory, based mostly on how these drugs behave rather than solid human studies, that cannabis might slow how your body clears barbiturates. In one small study THC stretched out how long a related barbiturate stuck around by about 4 hours.
In plain terms, mixing the two could leave more butalbital in your system and pile on the drowsiness. You might feel extra sleepy, foggy, dizzy, or slow to react, which is risky for driving. The evidence is thin, so don't panic, but please check with your pharmacist or doctor before using both together.

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).
All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
