Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist: your TB medication bedaquiline is cleared from your body by a liver enzyme called CYP3A4. Lab studies suggest cannabis may slow that enzyme down a bit. If that happens in real life, bedaquiline could build up slightly and its side effects might become more noticeable, things like nausea, headache, joint pain, or changes on a heart tracing (QT prolongation). Bedaquiline already carries a heart-rhythm warning, so that part is worth respecting.

That said, this is theoretical and based only on test-tube research, not human studies, so the real-world risk is uncertain. Because finishing your full MDR-TB regimen exactly as prescribed is so important, please loop in your doctor or pharmacist before mixing cannabis with bedaquiline.

About Bedaquiline

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
