Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

This combo product pairs chlorpheniramine (an antihistamine) with dextromethorphan (a cough suppressant). Both of these are handled in part by a liver enzyme called CYP3A4. The concern here is that cannabis may slow that enzyme down a bit, so these medicines could clear out of your body more slowly and build up slightly higher than usual.

In real life, that mostly means you might feel more drowsy, dizzy, foggy, or dry-mouthed than the label leads you to expect. I'll be honest, though: this is based only on lab studies, not on people, so it's more of a theoretical heads-up than a proven problem. Since both products already cause sleepiness, go easy, avoid driving, and check with your pharmacist or doctor first.

Here's the honest picture with this combo cold and cough product. The interaction on file says cannabis might speed up an enzyme called CYP2E1, which could in theory lower the levels of medicines that enzyme breaks down. But I want to be clear: this is based only on lab and animal research, not on real people, so it's very much theoretical. On top of that, the chlorpheniramine and dextromethorphan in this product are mainly handled by other enzymes (like CYP2D6), so CYP2E1 isn't their main path anyway.

What I'd actually keep an eye on is the overlap in effects. Both cannabis and chlorpheniramine can cause drowsiness, dizziness, and foggy thinking, and dextromethorphan can add to that. Mixing them may leave you more sedated than you expect. Don't drive, and please run it by your pharmacist or doctor first.
