Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that the cannabis constituents delta-9-tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol moderately inhibit the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's an interesting overlap to think about. Dronabinol is actually a man-made form of THC, one of the main compounds in cannabis. So combining the two is a bit like doubling up on the same thing, which can stack the usual effects: more drowsiness, dizziness, a faster heartbeat, mood changes, confusion, or that 'high' feeling.

On top of that, dronabinol is partly cleared by a liver enzyme called CYP2C9. Lab studies suggest cannabis compounds can mildly block that enzyme, which could let dronabinol build up a little and lead to stronger side effects. Keep in mind this enzyme part is theoretical and only based on test-tube research, not proven in people. Still, since you're essentially adding more cannabinoids, it's worth checking with your doctor or pharmacist before mixing them.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's something a little ironic: dronabinol (Marinol, Syndros) is actually a man-made version of THC, the same compound found in cannabis. So adding cannabis on top means you're essentially layering one cannabinoid onto another, which can stack up the effects you'd expect, like drowsiness, dizziness, feeling unusually 'high,' a fast heartbeat, or confusion.

On top of that, lab studies suggest cannabis may slow down the CYP3A4 enzyme that helps clear dronabinol from your body. In theory, that could let dronabinol build up and push those side effects higher. I'll be honest though, this part is based only on test-tube research, not real people, so the evidence is weak. Still, since both work on the same system, it's worth running this by your doctor or pharmacist before combining them.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
