Why this applies to Macitentan, Tadalafil

Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that the cannabis constituents delta-9-tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol moderately inhibit the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Opsynvi combines two blood pressure lowering drugs used for pulmonary arterial hypertension, and both rely partly on the liver enzyme CYP2C9 to be cleared out. Here's the concern: cannabis compounds like THC and CBD can gently slow that same enzyme. If it slows, these medicines may hang around a little longer than intended. That could mean stronger blood-pressure lowering effects, so you might notice more dizziness, lightheadedness, flushing, headache, or a stuffy nose (all common with tadalafil), plus possible swelling or low blood pressure from the macitentan side.

I want to be honest though, this warning comes only from lab studies, not real patients, so it's more of a theoretical caution than a proven problem. Still, before mixing cannabis with Opsynvi, please check in with your doctor or pharmacist so they can keep an eye on your blood pressure.

In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Opsynvi combines two medicines, macitentan and tadalafil, and both are broken down in part by a liver enzyme called CYP3A4. Lab studies (not human studies) hint that cannabis, especially CBD-rich products, might slow this enzyme down a bit. If that happened in your body, these drugs could build up more than usual.

What might that feel like? Since both lower blood pressure, you could notice more dizziness, lightheadedness, flushing, headache, or a stuffy nose, and tadalafil can add muscle aches or reflux. Keep in mind this is theoretical and only based on test-tube research, so it may not matter much in real life. Still, since Opsynvi treats pulmonary hypertension, it is worth a quick chat with your doctor or pharmacist before mixing in cannabis.
