Tofacitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabis interacts with.

Cannabis may increase levels of drugs metabolized by CYP2C19.
Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the gist for tofacitinib (Xeljanz). The CBD in cannabis can slow down a liver enzyme called CYP2C19, which is one of the routes your body uses to clear this medication. If that pathway gets blocked, tofacitinib can build up a bit higher in your blood, which may nudge up the chance of side effects like headaches, increased risk of infections, diarrhea, or changes in certain lab values (blood counts, cholesterol, liver tests).

I'll be honest with you: this concern is mostly theoretical, based on how the enzyme works rather than proven cases with tofacitinib specifically. Tofacitinib is the active drug itself, not a prodrug, so we'd expect more effect, not less. Still, since tofacitinib already affects your immune system, it's worth mentioning your cannabis use to your doctor or pharmacist so they can keep an eye on you.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Your body clears tofacitinib (Xeljanz) largely through a liver enzyme called CYP3A4. Lab studies suggest cannabis (especially CBD-rich extracts) can slow that enzyme down a bit. If that happened in real life, tofacitinib could build up in your system, which might mean stronger effects and a higher chance of side effects like headaches, increased infection risk, or changes in your blood counts and cholesterol.

Here's the honest part: this is only theoretical, based on test-tube research, not real patient cases. So it may not amount to much. Still, since tofacitinib already affects your immune system, it's worth mentioning your cannabis use to your pharmacist or doctor so they can keep an eye on you. Please don't change either one on your own.

What Is Tofacitinib Used For?

Brand names: Xeljanz, Xeljanz XR

  • Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
