Metformin, Sitagliptin falls under ANTIDIABETES DRUGS — the group Cod Liver Oil interacts with.

Theoretically, taking cod liver oil with antidiabetes drugs might increase the risk of hypoglycemia.
Some clinical research shows that cod liver oil may lower blood glucose levels in women with gestational diabetes who are already taking antidiabetes drugs when used for at least 12 weeks. However, it does not appear to have any effect on blood glucose when used for shorter durations. Until more is known, use caution.

Here's the gist with Janumet, which combines metformin and sitagliptin to lower your blood sugar. Cod liver oil isn't broken down by the same pathways as these medicines, so it doesn't really change how your body processes them. The concern is a bit more indirect: some research suggests that taking cod liver oil for a longer stretch (around 12 weeks or more) may nudge blood sugar down a little on its own, on top of what your diabetes medicine is already doing.

Stacked together, that could push your sugar lower than expected, leaving you shaky, sweaty, dizzy, or hungry. Honestly, the effect is modest and the evidence is mixed, but it's worth a heads-up to your doctor or pharmacist, especially if you keep an eye on your readings.
