Here's the gist for Selexipag (Uptravi): it works by relaxing the blood vessels in your lungs, and a known side effect is that it can nudge your blood pressure down. Cod liver oil, thanks to its omega-3 fatty acids, may also lower blood pressure a little. Stacked together, the two could have an additive blood-pressure-lowering effect.

In practice, that might mean more lightheadedness, dizziness, or feeling faint when you stand up. To be clear, this concern is largely theoretical and based on limited evidence, so it's not a reason to panic. Still, since Selexipag already tends to cause low blood pressure, it's worth mentioning to your pharmacist or doctor before adding cod liver oil so they can keep an eye on you.

Here's the thing to keep in mind with this pairing. The interaction on file is really about cod liver oil's mild effect on bleeding, not about how Selexipag (Uptravi) is broken down. In larger amounts (think 20 to 40 mL a day), the omega-3s in cod liver oil can make platelets a little less sticky and stretch out bleeding time. Selexipag itself relaxes blood vessels in your lungs and can already nudge bleeding risk slightly, so stacking the two together could, in theory, raise that risk a bit.

The evidence here is fairly modest, and small food-level amounts are unlikely to cause trouble. Still, if you notice easy bruising, nosebleeds, or bleeding that won't quit, let your pharmacist or doctor know before changing anything.

What Is Selexipag Used For?

Brand name: Uptravi

Selexipag tablets are used to treat pulmonary arterial hypertension (PAH, WHO Group I) — a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to reduce the chances of being hospitalized because of PAH.

Uptravi (selexipag) tablets are approved for both adults and children aged 2 years and older. Uptravi for injection (intravenous) is also available for adult patients who are already on Uptravi tablets but temporarily cannot take medicine by mouth.
