Bupivacaine Hci falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the idea behind this pairing. Bupivacaine is a local anesthetic, often used to numb a surgical area or as a nerve block. At high blood levels, local anesthetics like bupivacaine can irritate the nervous system and, in rare cases, trigger seizures. Dendrobium contains a compound called dendrobine that, in lab and animal work, has shown convulsant (seizure-promoting) effects.

So the worry is that taking the two together could theoretically nudge the seizure threshold lower and make a seizure slightly more likely. I want to be honest though: this is a theoretical concern based on pharmacology, not real-world reports. Still, since you usually receive bupivacaine in a medical setting, it's worth telling your surgeon, anesthesia team, or pharmacist that you take Dendrobium so they can weigh in.
