Why this applies to Bupivacaine, Meloxicam

Bupivacaine, Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Zynrelef combines two medicines applied at a surgical site: bupivacaine, a numbing agent, and meloxicam, an anti-inflammatory. The piece that matters here is meloxicam, which your liver breaks down using an enzyme called CYP2C9. Diosmin may gently slow that same enzyme, so in theory meloxicam could clear a little more slowly and hang around at slightly higher levels.

If that happens, you might be more prone to typical meloxicam side effects like stomach upset, heartburn, fluid retention, or, less often, blood pressure or kidney effects. Keep in mind this is theoretical, based on how the drugs behave rather than strong human studies, and Zynrelef is a one-time, in-procedure product. Still, it is worth mentioning diosmin to your surgeon or pharmacist beforehand.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin.
