Why this applies to Naproxen Sodium

Naproxen Sodium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the gist: your body relies on a liver enzyme called CYP2C9 to clear naproxen out of your system. Diosmin may slow that enzyme down a bit. If naproxen leaves your body more slowly, it can build up, and that means the pain-relieving effect, plus the usual naproxen side effects, could be stronger than expected.

With naproxen, the side effects to watch are stomach upset, heartburn, a higher chance of stomach irritation or bleeding, fluid retention, and effects on blood pressure or the kidneys. That said, this interaction is theoretical, based mostly on how a related drug (diclofenac) behaves, not on naproxen itself. Occasional use is unlikely to cause trouble for most people, but if you take both regularly, check with your pharmacist or doctor first.

Theoretically, diosmin may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Here's the gist: naproxen (the pain reliever in Midol Extended Relief and Aleve) already nudges up your bleeding risk on its own. It thins the blood a little by blocking platelets, and it can be rough on the stomach lining, which is why some people get heartburn, stomach pain, or even ulcers. Diosmin may also have a mild effect on platelets, so stacking the two could, in theory, add up to a bit more bleeding tendency.

I want to be honest about how thin the evidence is here. This is mostly theoretical, leaning on a single case report that also involved warfarin and aspirin, not naproxen alone. For most healthy folks taking occasional naproxen, this likely isn't a big deal. Still, watch for unusual bruising, nosebleeds, dark stools, or stomach upset, and check with your pharmacist or doctor before combining them regularly, especially if you take any other blood thinner.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies.
