Why this applies to Tazemetostat Hydrobromide

Tazemetostat Hydrobromide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist: Tazemetostat (Tazverik) is cleared from your body by a liver enzyme called CYP3A4, and diosmin may slow that same enzyme down. If diosmin blocks it, your body could break down tazemetostat more slowly, letting levels creep up. That could mean more of the usual tazemetostat side effects, like fatigue, nausea, low blood counts, or a higher risk of infection.

I want to be honest about the evidence though. This is mostly theoretical, and lab results are mixed. The clearest data comes from a different drug (carbamazepine), not tazemetostat. With a cancer medicine like this, I wouldn't guess. Please check with your oncologist or pharmacist before adding diosmin so they can keep you safe.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Tazemetostat Used For?

Brand name: Tazverik

Tazverik (tazemetostat) tablets are used to treat adults and patients aged 16 and older with metastatic or locally advanced epithelioid sarcoma (a rare soft-tissue cancer) that cannot be completely removed by surgery.

Tazverik is also used in adults with relapsed or refractory follicular lymphoma — meaning the lymphoma has come back or stopped responding to treatment — either when the tumor carries a specific EZH2 gene mutation (confirmed by an FDA-approved test) after at least two prior treatments, or when there are no other satisfactory treatment options. Both uses were approved based on response rates, and continued approval may depend on results from ongoing confirmatory studies.
