In vitro research shows that equol inhibits the binding of estradiol to serum proteins. Theoretically, this might increase the concentration of free, bioactive estradiol, thereby increasing the risk of adverse effects from estrogen replacement therapy, including headaches, breast tenderness, and weight gain.

Activella combines estradiol (an estrogen) with norethindrone, and it's often used to ease menopause symptoms. Here's the gist of this pairing: equol is a plant compound that gently mimics estrogen. In lab studies, it appears to bump estradiol off the proteins that normally carry it in your blood. The thought is that this could leave a little more free, active estradiol floating around.

If that happened, you might notice more of estradiol's usual effects, things like headaches, breast tenderness, or some water weight. But honestly, this is theoretical and only seen in test tubes, not in people, so I wouldn't lose sleep over it. Soy from food is unlikely to matter. Before adding an equol supplement, just run it by your pharmacist or doctor first.
