Theoretically, equol might increase the levels and clinical effects of estrogens.
In vitro research shows that equol inhibits the binding of estradiol to serum proteins. Theoretically, this might increase the concentration of free, bioactive estradiol, thereby increasing the risk of adverse effects from estrogen replacement therapy, including headaches, breast tenderness, and weight gain.

Here's the idea behind pairing equol with esterified estrogens like Menest or Estratab. Equol is a plant compound that gently acts like estrogen, but the bigger concern from lab studies is that it may bump some of your estrogen off the proteins it normally rides on in your blood. When estrogen is freed up that way, more of it becomes active, which could nudge up your estrogen-related effects.

In real life that might show up as headaches, breast tenderness, or some weight gain. I want to be honest though: this comes only from test-tube research, not from people, so it's theoretical and may not amount to much. Still, since you're already on a full estrogen dose, it's worth a quick chat with your pharmacist or doctor before adding equol rather than starting it on your own.
