Prasterone (prescription drug) falls under ESTROGENS — the group Equol interacts with.

Theoretically, equol might increase the levels and clinical effects of estrogens.
In vitro research shows that equol inhibits the binding of estradiol to serum proteins. Theoretically, this might increase the concentration of free, bioactive estradiol, thereby increasing the risk of adverse effects from estrogen replacement therapy, including headaches, breast tenderness, and weight gain.

Here's the gist with Intrarosa (prasterone): your body naturally converts this medicine into small amounts of estrogen and other hormones right where it's applied. Equol, a soy-based phytoestrogen, has been shown in test-tube studies to bump estrogen off the proteins that carry it in your blood. In theory, that could leave a little more active estrogen floating free, which might nudge up estrogen-type side effects like breast tenderness, headaches, or some fluid retention.

I want to be honest about the strength of this, though. It's based only on lab research, not real people, so it's quite theoretical. With vaginal prasterone the hormone exposure is fairly low to begin with. Still, before you pair equol with Intrarosa, run it by your pharmacist or prescriber to be safe.
