Aprocitentan falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ergot interacts with.

Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's the thing to understand about this pairing. Ergot (through its alkaloid ergotamine) is cleared by a liver enzyme called CYP3A4. If something slows that enzyme down, ergot can pile up in your body, and ergot toxicity is no joke. It can cause severe blood vessel narrowing, leading to cold or painful hands and feet, numbness, and in bad cases tissue damage.

Now, aprocitentan (Tryvio) is a blood pressure medicine, and it can act as a mild CYP3A4 inhibitor. So in theory it could let ergot build up a bit. I'll be honest with you though, this concern is theoretical and based on weak, anecdotal evidence. Honestly, the bigger issue is that crude ergot itself isn't safe to take at all. Please talk with your pharmacist or doctor before combining them.

What is Ergot?

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

What Is Aprocitentan Used For?

Brand name: Tryvio

Tryvio is used to treat high blood pressure (hypertension) in adults. It is specifically for people whose blood pressure is not adequately controlled on other blood pressure medications — it is always used together with at least one other antihypertensive drug, not on its own.

Lowering blood pressure helps reduce the risk of serious cardiovascular events such as stroke and heart attack. Tryvio has not been studied as a standalone treatment and is not intended to replace other blood pressure medicines.
