Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the gist for these two together. Clarithromycin (Biaxin) is a strong blocker of a liver enzyme called CYP3A4, which is one of the main pathways your body uses to break down evodia's active compounds. When that pathway is slowed down, evodia can hang around longer and build up to higher levels than usual. So in this pairing, it's the evodia that may become more potent, not the antibiotic.

I want to be honest about the strength of this: it comes from lab and animal studies, not from people, so it's more theoretical than proven. Higher evodia levels could mean more of its effects, like stomach upset or changes in body temperature. Since clarithromycin is only taken for a short course, this is usually short-lived, but it's still worth running by your pharmacist or doctor first.

Here's the gist for clarithromycin (Biaxin) and Evodia. Clarithromycin can block a liver enzyme called CYP1A2, which is the same enzyme that helps break down one of Evodia's active compounds (rutaecarpine). So the concern isn't about clarithromycin itself building up. Instead, the worry runs the other way: clarithromycin could slow Evodia's clearance, letting more of the Evodia linger in your body and possibly boosting its effects or side effects.

I'll be honest, this is mostly theory from lab and animal research, not real patient reports, so the practical risk is probably low. Still, since clarithromycin is a short course for an infection, it's easy to simply pause the Evodia while you're on it. Check with your pharmacist or doctor before combining them.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the thing to know about combining evodia with clarithromycin: clarithromycin is one of those antibiotics that can stretch out the heart's electrical reset time, something we call QT prolongation. In rare cases that can trigger a dangerous irregular heartbeat. Evodia, based on lab and animal studies, appears to do the same kind of thing. Stacking two QT-affecting substances together could, in theory, add up and raise that risk.

I'll be straight with you: this concern comes from test tube and animal research, not from people, so it's more of a caution than a proven problem. Still, since clarithromycin alone already carries some QT risk, I wouldn't pair it with evodia without checking first. Please run it by your pharmacist or doctor before using both.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
