Trandolapril, Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Tarka combines two blood pressure medicines: trandolapril (an ACE inhibitor) and verapamil (a calcium channel blocker). The concern on file is that Evodia may speed up a liver enzyme called CYP1A2, which can lower the levels of drugs that depend on it. In theory, that could make this combination work a little less well, so you might see your blood pressure creep up or your heart rate rise.

Honestly though, this evidence is weak. It comes from test tube and animal studies, and verapamil and trandolapril aren't mainly handled by CYP1A2 (verapamil leans more on CYP3A4). So a big effect is unlikely. Still, since blood pressure control matters, please check with your pharmacist or doctor before adding Evodia, and keep an eye on your readings.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing to know about Tarka (trandolapril plus verapamil): the verapamil part can block a liver enzyme called CYP3A4. That same enzyme helps break down evodia's active compounds. So if you take them together, evodia could build up higher than usual in your body. In this case it's evodia whose levels go up, not your blood pressure medicine.

What does that mean for you? Honestly, the evidence here is weak. It comes from lab and animal studies, not real patients, so we can't say how big a deal it is in people. It's more that evodia's effects (and any side effects) might be stronger than expected. Since you're on a blood pressure medicine, it's worth a quick chat with your pharmacist before adding evodia.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Tarka is a combo pill: trandolapril (a blood pressure medicine) plus verapamil. The piece that matters here is verapamil, which your liver clears using an enzyme called CYP3A4. In lab dishes, evodia appears to slow that same enzyme down. If that held true in people, verapamil could build up a bit more than usual, nudging its effects stronger. With verapamil that can mean too-low blood pressure, dizziness, a slow heart rate, constipation, or swelling in the ankles.

The important caveat: this is only test-tube and animal evidence, and it has not been seen in real patients, so I wouldn't lose sleep over it. Still, since heart and blood pressure medicines leave little room to spare, please run evodia past your pharmacist or doctor before combining them.
