Garcinia and Chlorpheniramine, Phenylephrine, Pyrilamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Chlorpheniramine, Phenylephrine, Pyrilamine

Chlorpheniramine, Phenylephrine, Pyrilamine falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.

Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination cold and allergy product mixes an antihistamine (chlorpheniramine), a decongestant (phenylephrine), and a second antihistamine (pyrilamine). The concern here is that chlorpheniramine has mild serotonin-boosting activity, and Garcinia (through its hydroxycitric acid) may nudge serotonin levels up too. Stacking the two could, in theory, raise your odds of serotonin syndrome, which can show up as agitation, a racing heart, sweating, shivering, tremor, or confusion.

I want to be honest about the evidence though: this is largely theoretical and based on a single case report involving an antidepressant, not this cold medicine. For most people taking Tanamine occasionally, the real-world risk is low. Still, since Garcinia's benefits are weak and unproven, it's worth running it by your pharmacist or doctor first, especially if you have liver concerns.

From our pharmacist’s overview on the Chlorpheniramine, Phenylephrine, Pyrilamine page

“Tanamine pairs two antihistamines, chlorpheniramine and pyrilamine, with phenylephrine, a decongestant that raises blood pressure and heart rate, and that mix is exactly why I treat its supplement list as high-stakes. Ten of the 229 ingredients on file rate major, and they belong at the top of your reading.”
