Garcinia and Maraviroc

Here is the interaction we found between these two, with the evidence behind it.

HEPATOTOXIC DRUGS

Why this applies to Maraviroc

Maraviroc falls under HEPATOTOXIC DRUGS — the group Garcinia interacts with.

Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage.
There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern when you pair Garcinia with maraviroc: both can be hard on the liver. Maraviroc already carries a warning about liver injury, and Garcinia has its own case reports of acute hepatitis and rising liver enzymes. Stacking two things that stress the same organ may add up. This isn't about Garcinia changing how much maraviroc is in your blood, it's about a shared side effect.

To be fair, this is a theoretical concern based on how each is known to behave, not proven in studies of the two together. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Please check with your pharmacist or doctor before adding Garcinia, and don't change your HIV medicine on your own.

From our pharmacist’s overview on the Maraviroc page

“Someone taking maraviroc (Selzentry) for HIV needs a steady blood level to keep the virus suppressed, and that level is exactly what most of the 168 supplements on this page can disturb, so this list calls for genuine care. About 121 entries share maraviroc's clearance route, the CYP3A4 enzyme, which means grapefruit can push levels up…”

About Garcinia

What is Garcinia?

Garcinia
  • Weight loss
  • Appetite suppression
  • Curbing sugar and carb cravings
  • Supporting metabolism
  • Traditional cooking and souring agent

Garcinia is a tropical fruit whose rind contains hydroxycitric acid (HCA), widely marketed for weight loss and appetite control. The scientific evidence is weak and mixed, with most studies showing only small or no meaningful weight effects, and rare reports of liver problems raise safety concerns. Talk to a pharmacist or doctor before trying it, especially if you have liver issues, are pregnant, or are breastfeeding.

About Maraviroc

What Is Maraviroc Used For?

Brand names: Celsentri, Selzentry

Maraviroc treats a specific type of HIV-1 infection called CCR5-tropic HIV-1. It is always used alongside other antiretroviral (anti-HIV) medicines — never alone. A special test called a tropism assay must be done before starting treatment to confirm you have the right type of HIV.

Selzentry (the brand-name oral solution and tablet) is approved for adults and pediatric patients weighing at least 2 kg. Generic maraviroc tablets are approved for adults and pediatric patients aged 2 years and older weighing at least 10 kg. Neither form is recommended for patients whose HIV is classified as dual/mixed-tropic or CXCR4-tropic.
