Garcinia and Ziprasidone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Ziprasidone

Ziprasidone falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.

Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thinking behind pairing Garcinia with ziprasidone (Geodon). Ziprasidone gently nudges up serotonin activity in the brain as part of how it works. Garcinia is believed, on a theoretical level, to also raise serotonin a bit. Stack the two and you could, in rare cases, get too much serotonin signaling, which is what we call serotonin syndrome.

I want to be honest about how shaky this is. The concern comes from a single case report involving Garcinia plus an antidepressant, not ziprasidone, so it's really a theoretical worry. Still, watch for things like agitation, fast heartbeat, sweating, shaking, or muscle twitching. Please loop in your pharmacist or doctor before starting Garcinia, especially since its weight-loss benefits are weak and unproven.

From our pharmacist’s overview on the Ziprasidone page

“Ziprasidone (Geodon) is an antipsychotic that already carries a warning about disturbing the heart's electrical rhythm, so a list with 11 major-rated interactions calls for real care before mixing in supplements. Most of the 188 ingredients on file touch the liver enzyme, CYP3A4, that clears ziprasidone from the body.”
