Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.

Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.

This cold and pain combo (acetaminophen, chlorpheniramine, phenylephrine, and salicylamide) isn't a classic serotonin booster the way an antidepressant is. The concern on file is mostly theoretical, based on a couple of anecdotal reports where Garcinia was paired with strong serotonergic medicines. None of the ingredients in Rhinogesic are big drivers of serotonin, so I wouldn't expect a real-world problem from this pairing in most people.

That said, chlorpheniramine and phenylephrine can already cause things like drowsiness, dry mouth, a racing heart, or feeling jittery, so be aware of those on their own. Since Garcinia's evidence is weak and it carries its own liver worries, please run it past your pharmacist or doctor before combining them.

There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.

This combo cold and pain product (Rhinogesic) contains acetaminophen and salicylamide, both of which your liver has to process. Acetaminophen in particular is well known for stressing the liver when taken in high doses or for long stretches. Garcinia has its own rare reports of liver irritation, with cases of hepatitis and elevated liver enzymes. So the worry here is simple: putting two things that can each tax the liver together might add up.

To be fair, this concern is mostly theoretical and based on how these ingredients behave, not on strong studies. There's no enzyme blocking that makes the medicine build up. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before pairing them, especially if you have any liver concerns.
