Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Garcinia interacts with.

Theoretically, hydroxycitric acid (HCA), the main active ingredient in garcinia, might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
HCA inhibits platelet aggregation in vitro. The inhibitory effect seems to be greater in platelets extracted from diabetic subjects than non-diabetic subjects.

Here's the thing to know about combining Garcinia with alteplase (tPA). Alteplase is a powerful clot-busting medicine given in the hospital during emergencies like a stroke, heart attack, or serious clot. It works fast to break up clots, and its biggest known risk is bleeding. Garcinia's active compound, HCA, has shown in lab dishes that it can make platelets a bit less sticky, so in theory it could add to bleeding risk.

That said, this is a theoretical concern only, based on test-tube studies, not real patient cases. And practically speaking, alteplase is an emergency drug your medical team controls. If you take Garcinia regularly, it's still worth mentioning to your doctor or pharmacist so your full picture is known.
