SEROTONERGIC DRUGS
Why this applies to methylphenidate transdermal system
methylphenidate transdermal system falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for the Daytrana patch and Garcinia. Methylphenidate isn't a serotonin drug in the classic sense, but as a stimulant it nudges up brain chemicals (including some serotonin activity). The worry on file is that Garcinia might add a little extra serotonin push, and stacking serotonin-raising things can, in rare cases, lead to serotonin syndrome: agitation, a racing heart, sweating, shakiness, high fevers, or confusion.
I'll be honest with you: this is mostly theoretical, based on a single report involving an antidepressant, not the Daytrana patch. The evidence is thin. Still, since Garcinia's weight-loss benefit is weak and unproven, and it carries its own liver concerns, I'd run it past your pharmacist or doctor before combining the two. Don't change your patch on your own.

From our pharmacist’s overview on the methylphenidate transdermal system page
“Daytrana, the methylphenidate skin patch for ADHD, is itself a stimulant, and that shapes nearly this whole list, which I read as one that calls for real care. Of the 68 supplement ingredients on file, 44 are stimulants too, so pairing them can push heart rate and blood pressure higher than either alone.”
