Garcinia and Alosetron

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Alosetron

Alosetron falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.

Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture: the worry on file is that Garcinia might add to the serotonin activity of certain medicines and, in rare cases, tip someone toward serotonin syndrome. The catch is that this concern comes from a couple of case reports involving antidepressants, not from Alosetron specifically. Alosetron actually works by blocking a serotonin receptor (the 5-HT3 type) to slow down your gut, so it isn't a typical serotonin-boosting drug.

So for Alosetron, this interaction is largely theoretical and the evidence is weak. I wouldn't expect a big problem, but I also can't promise zero risk. If you ever feel agitated, shaky, sweaty, feverish, or have a racing heart, get help right away. Please check with your pharmacist or doctor before adding Garcinia, especially since it has been linked to liver issues.

From our pharmacist’s overview on the Alosetron page

“Alosetron (Lotronex) treats severe diarrhea-predominant IBS by acting on serotonin receptors in the gut, and that is where most of its 38 supplement interactions come from. Twenty of the ingredients, 5-HTP among them, raise serotonin on their own, and stacking them on a serotonin-acting drug is a combination I want you to treat with re…”
