Acetaminophen, Butalbital, Codeine Phosphate falls under CNS DEPRESSANTS — the group Goldenseal interacts with.
Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill already has two ingredients that calm the nervous system: butalbital (a barbiturate) and codeine (an opioid). Both can make you drowsy, foggy, and slow your breathing. The concern with goldenseal is that the berberine it contains has shown sedative effects in animal studies, so in theory it could add to that sleepiness.
I want to be honest about how soft this evidence is. This effect has only been seen in animals, not in people, so it's a theoretical caution rather than a proven problem. Still, if you ever feel extra groggy, unsteady, or unusually short of breath when combining them, that's worth paying attention to. Before adding goldenseal, check with your pharmacist or doctor so they can look at your full medication list.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 3Acetaminophen, Butalbital, Codeine Phosphate also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with this combo. The codeine part of Phrenilin with Codeine has to be switched on by a liver enzyme called CYP2D6, which turns it into morphine, the form that actually relieves pain. Goldenseal can block that same enzyme. So instead of building codeine up, goldenseal may leave less of it converted into the active painkiller, meaning you could get weaker pain relief.
The evidence here is moderate, not rock solid, and the acetaminophen and butalbital parts aren't the main concern. Still, if your headache pain isn't being controlled, that's worth noticing. Don't stop either one on your own. Just check with your pharmacist or doctor before pairing goldenseal supplements with this medication.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that goldenseal root extract can inhibit the activity of CYP2E1. However, this effect has not been reported in humans.

This combination product has three parts, but the piece that lines up with goldenseal is the acetaminophen (Tylenol). One pathway your liver uses to process acetaminophen is an enzyme called CYP2E1. In lab dish studies, goldenseal seemed to slow that enzyme down a bit. In theory, slowing it could leave a little more acetaminophen circulating in your blood.
Here is the honest part: this has only been seen in test tubes, never in real people, so it is mostly a theoretical concern. Interestingly, that pathway is the one that creates acetaminophen's liver-harming byproduct, so blocking it would not clearly make things more dangerous. Still, the safest move is to keep your total acetaminophen reasonable and let your pharmacist or doctor know you take goldenseal before combining them.
