Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

Here's the simple version: Xanax (alprazolam) already calms the brain and can leave you feeling drowsy, foggy, or unsteady. Goldenseal contains berberine, and in animal studies that compound showed some sedative activity of its own. The concern is that stacking the two could, in theory, add up to more sleepiness, slowed thinking, or poor coordination.

I want to be honest about the strength of this one. This interaction is theoretical, based on lab and animal data, and the added sedation hasn't actually been reported in people. So it's not a reason to panic. Still, if you're taking Xanax and thinking about goldenseal, run it by your pharmacist or doctor first, and notice any extra drowsiness, especially before driving.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Alprazolam also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the thing to know if you take Xanax (alprazolam) and are thinking about goldenseal. Your liver and gut use an enzyme called CYP3A4 to break down alprazolam and clear it out. Goldenseal (mainly its berberine and hydrastine) tends to block that enzyme, so the medication can clear more slowly and build up in your blood.

In practical terms, that could mean a stronger or longer-lasting effect than expected: more drowsiness, dizziness, foggy thinking, or feeling unsteady. The research here is moderate, not certain, but it's a real enough signal that I wouldn't ignore it. Please don't stop or change your alprazolam on your own. Just check with your pharmacist or doctor before adding goldenseal so they can weigh it for you.

What Is Alprazolam Used For?

Brand names: Xanax, Niravam

Alprazolam treats generalized anxiety disorder (GAD) and panic disorder, with or without agoraphobia, in adults. These uses are supported across multiple alprazolam products, including tablets, orally disintegrating tablets, oral solution, and extended-release tablets.

Xanax (tablet) is indicated for the acute treatment of GAD and for panic disorder. Xanax XR (extended-release tablet) and alprazolam extended-release tablets are indicated specifically for panic disorder with or without agoraphobia — not for GAD. Alprazolam orally disintegrating tablets and alprazolam oral solution are indicated for both GAD and panic disorder.
