Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

Fiorinal with Codeine is already a sedating combo, mostly because of the butalbital (a barbiturate) and the codeine. Both can leave you drowsy, foggy, or lightheaded on their own. The concern here is simple: goldenseal contains berberine, and in animal studies berberine showed some sedative effects. So in theory, stacking it with this medication could add to the sleepiness, slow breathing, or that 'out of it' feeling.
I want to be honest though, this drowsiness from goldenseal hasn't actually been reported in people, so this is more of a precaution than a proven problem. Still, since this medicine can already knock you out, it's worth running goldenseal by your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with this combination pill: the codeine portion is actually a prodrug. Your body has to use an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Goldenseal slows that same enzyme down, so less codeine gets turned into morphine. The likely result is that you get less pain relief from the codeine, not more.
The aspirin, butalbital, and caffeine parts aren't really affected by this pathway. So the main concern is weaker relief from the codeine, which could leave your headache or pain undertreated. Food amounts of goldenseal are unlikely to matter, but concentrated supplements can. Before mixing them, check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

This combo pill has a few moving parts, but the one I'd watch with goldenseal is the butalbital, a barbiturate that the liver clears partly through CYP3A4. Goldenseal tends to slow that enzyme down, so butalbital could linger longer than usual. That might mean extra drowsiness, dizziness, or feeling groggy and foggy, especially if you take it regularly through the day.
There's a second wrinkle. The codeine here is a prodrug that the body has to switch on (mainly through a different enzyme, CYP2D6) to relieve pain. Goldenseal mostly hits CYP3A4, so its effect on the codeine side is less clear. The evidence here is moderate, not alarming, but I'd still loop in your pharmacist or doctor before pairing the two.

This combination product contains aspirin, which already thins your blood a bit by keeping platelets from clumping together. Goldenseal's main compound, berberine, has shown a similar platelet-slowing effect in lab dishes and animal studies. So in theory, pairing the two could add up and raise your risk of bleeding or bruising more easily.
Here's the honest part: this concern comes only from test-tube and animal research, and it hasn't actually been seen in people. So I wouldn't lose sleep over it, but it's worth keeping an eye out for things like easy bruising, nosebleeds, or bleeding gums. If you take goldenseal regularly alongside this medicine, just give your pharmacist or doctor a heads-up so they can keep watch.
