Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist with Evotaz (atazanavir plus cobicistat) and goldenseal. Your body relies on an enzyme called CYP3A4 to break down atazanavir, and the cobicistat in this combo is actually there to slow that enzyme down on purpose so atazanavir levels stay high enough to work. Goldenseal, mainly its berberine and hydrastine, also blocks CYP3A4. Stacking another blocker on top could nudge atazanavir levels higher than intended.

If that happens, you might notice more of atazanavir's usual side effects, like yellowing of the eyes or skin (from raised bilirubin), nausea, or stomach upset. The evidence here is moderate and not certain for this exact drug, so don't stop anything. Just check with your pharmacist or HIV provider before adding goldenseal.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
