Bisoprolol falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: bisoprolol already lowers your blood pressure and slows your heart rate. Goldenseal contains berberine, and the concern is that it might nudge your blood pressure down a little further. If both pull in the same direction, you could feel light-headed, dizzy, unusually tired, or notice your pulse running slow, especially when you stand up quickly.

I want to be honest about how soft this evidence is. It's theoretical, based mostly on animal studies and a separate blood pressure herb (berberine with amlodipine), not goldenseal itself in people taking bisoprolol. So this isn't a reason to panic. Still, if you'd like to try goldenseal, run it by your pharmacist or doctor first and keep an eye on how you feel.

Here's the gist with goldenseal and your bisoprolol. Goldenseal's berberine can slow down a liver enzyme called CYP2D6, which helps your body break down certain medications. Bisoprolol is partly cleared this way (it's the active drug already, not a prodrug that needs switching on), so blocking that pathway can let a bit more of it linger in your bloodstream.

In plain terms, that could mean a stronger beta-blocker effect than usual: a slower heart rate, lower blood pressure, dizziness, lightheadedness, or feeling unusually tired. The good news is bisoprolol leans more on your kidneys to leave the body, so the effect may be modest, and the evidence here is moderate quality. Still, please check with your pharmacist or doctor before pairing them rather than adjusting anything on your own.
