Bivalirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Goldenseal contains berberine. In vitro and animal research shows that berberine can inhibit platelet aggregation. However, this effect has not been reported in humans.

Here's the short version: Bivalirudin is a powerful blood thinner given through an IV during heart procedures, like angioplasty or stent placement. It works directly to block clotting, so you'd only ever get it in a hospital where a team is watching you closely. Goldenseal contains berberine, which in lab dishes and animal studies seemed to make platelets a little less sticky. In theory, stacking that on top of bivalirudin could nudge your bleeding risk higher.

But here's the honest part: this effect has never actually been shown in people, so it's a theoretical concern, not a proven one. Since bivalirudin is used in a monitored setting, your care team will already know your full medication and supplement list. Just be sure to mention any goldenseal so they have the full picture.
