Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist for Marinol or Syndros (dronabinol): your liver uses an enzyme called CYP2C9 to help clear this medicine. Lab studies suggest goldenseal can mildly slow that same enzyme. If that happened in your body, dronabinol could clear a little slower and build up, which might mean stronger or longer-lasting effects like drowsiness, dizziness, a racing or pounding heartbeat, confusion, or feeling more 'high' than expected.

The important part: this is only a theoretical concern seen in test tubes, not something proven in real people. So I wouldn't panic. Dronabinol isn't a prodrug, so the direction (more effect, not less) makes sense if anything happens at all. Still, run it by your pharmacist or doctor before combining them, especially if you're new to dronabinol.

Dronabinol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down dronabinol. Goldenseal appears to slow that enzyme down. When the cleanup crew works more slowly, more dronabinol can linger in your system than expected.

For a drug like dronabinol, that usually means its effects get turned up. You might notice more drowsiness, dizziness, a faster heartbeat, confusion, or feeling unusually high or anxious. This concern comes from real but limited human research, so it's labeled possible, not certain. Everyday culinary amounts aren't the issue here; concentrated goldenseal supplements are what matter. Please don't change anything on your own. Just check with your pharmacist or doctor before combining the two.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
