Fosamprenavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the thing to know about pairing goldenseal with Lexiva (fosamprenavir). Goldenseal tends to block a liver and gut enzyme called CYP3A4, and that same enzyme helps clear fosamprenavir from your body. Slow down that clean-up crew, and the drug can build up higher than expected.

In real terms, that could mean a greater chance of side effects like nausea, diarrhea, headache, rash, or unusual fat changes. Now, this is mostly based on lab work and small studies, not big trials, so it's a possible concern rather than a sure thing. Still, HIV medicines are too important to gamble with, so please check with your pharmacist or doctor before adding goldenseal rather than starting or stopping anything on your own.

What Is Fosamprenavir Used For?

Brand names: Lexiva, Telzir

Fosamprenavir tablets (sold as Fosamprenavir Calcium and Lexiva) are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children as young as 4 weeks old. It is not used alone — it is always part of a combination regimen.

For adults who have already taken protease inhibitor medicines before, fosamprenavir is used alongside a low dose of ritonavir. Once-daily dosing with ritonavir is not recommended for adults who have previously used protease inhibitors, or for any pediatric patients.
